Our Population App helps lower healthcare costs by allowing patients to better engage, and take control, of their health decisions by utilizing self-generated data. Patients gather information by connecting existing devices (i.e. glucose monitors, smart watches, activity trackers (fitbit) and health apps to their personal dashboards to see progress and next steps in a simple digestible format. In addition to data, patients have on-demand access to past online and in-person consult histories with clinician notes that can be shared anytime.
